Confidentiality and Data Security:

  1. All client informations and data will be kept confidential and stored securely in our head office under a double lock.

Initial Assessment:

  1. An initial assessment involves an in-person or virtual consultation with family members to discuss the client’s needs.
  2. Client information provided during the assessment establishes a baseline for comparison and better understands of the client’s requirements.

LTC Navigation Packages:

  1. Families are required to provide a list of five LTC facilities of their choice and any supporting documentation.
  2. Clients should understand that admission to one of their selected facilities may not be guaranteed if the client’s health condition is affected. If the client is impacted by  Bill 7, More Beds, Better Care Act, 2021, lf the client is hospitalized prior to admission or a different placement decision.

Report Generation:

  1. For the Golden Package, please allow a 72-hour turnaround time for report generation.
  2. For the Exclusive Package, a minimum of 7 days is needed for report generation, subject to the availability of LTC homes for tours.

Onsite Visits:

  1. Onsite visits are scheduled after confirming availability with LTC homes.
  2. Challenges affecting availability include outbreaks, staff shortages, inspections, or accreditation processes. This will delay report generation. Open communication will be ongoing between the company and the client to ensure they remain informed. Alternatively, reports can be generated for the other homes of choice that are not affected within the respected time frame if the house is not subject to the above challenges.

Consulting Services:

  1. Consulting services are charged at a flat rate of $125 per hour, with additional time billed in 30-minute increments.
  2. Payment must be made and appointments booked before receiving consulting services.

Pricing for Healthcare Services:

  1. Pricing for healthcare services varies based on project duration, the number of employees, home size (bed count), and the need for evening or night work.

Intellectual Property:

  1. All intellectual property rights for resource materials, training content, and workshops belong to Mildred Ababio Consulting Inc.

Licensing and Regulatory Compliance:

  1. All consultants must provide proof of nursing licenses and demonstrate good standing with the College of Nursing of Ontario.

  • Pricing and Price Changes:
  1. The prices for our health consultation services are subject to change without prior notice. We reserve the right to adjust our pricing at any time.
  2. Changes to prices will be effective immediately upon being posted on our website or communicated to clients through other means.
  3. Clients who have already engaged in our services prior to a price change will not be affected by the change for the duration of their current engagement.
  4. New clients and clients who renew their services after a price change will be subject to the updated pricing.
  • Subscription to “Wellness Check” Service:
  1. Clients may subscribe to the “Wellness Check” service after an initial assessment of their needs.
  2. The service costs $300 per month (1 visit per month), with the option to increase the frequency of the visit. It is essential to note that this is an ongoing service, and the client understands it is a monthly subscription. Services can be canceled anytime as long as they follow our cancellation policy.
  • LTC Navigation Packages:
  1. The Golden Package costs $690 and includes a comprehensive homes report for five selected homes.
  2. The Exclusive Package costs $897 and offers onsite visits to these homes with additional features.
  • Cancellation and Refund Policy:
  1. There are no refunds for the Golden and Exclusive Packages.
  2.  Clients must make all cancellations in writing 30 days in advance for Wellness Checks, LTC services and Healthcare Services.
  3. Mildred Ababio Consulting reserves the right to cancel its services at any time without prior notice and without incurring any consequences or liabilities.
  4. In the event of service cancellation, Mildred Ababio Consulting. will not be held responsible for any resulting damages, losses, or inconveniences.
  5. Clients or users affected by the cancellation of services will not be entitled to compensation, refunds, or remedies from Mildred Ababio Consulting Inc.
  • Intellectual Property:

  2. The consultant serves as a liaison between the client’s family, caregivers, and the LTC facility, facilitating effective communication and understanding.
  3. It is important to note that the consultant will never make derogatory statements about LTC facilities or imply that they are non-compliant with industry standards.
  4. The consultant’s role is not that of a compliance inspector or regulatory officer. Instead, they focus on supporting the LTC facility and advocating for the client.
  5. The consultant’s aim is to assist the client in receiving the highest standard of care, resulting in better patient outcomes and an improved quality of life.
  • 18.Respect for LTC Privacy Policies and Reporting Negligence:
  1. The consultant is committed to respecting the Long-Term Care (LTC) facility’s privacy policies and practices in accordance with applicable laws and regulations.
  2. In the event the consultant becomes aware of any negligent act or wrongdoing within the LTC facility that may impact the safety or well-being of the client, the consultant has a duty to inform the client promptly.
  3. The consultant is also subject to reporting such acts or wrongdoings as required by law, including but not limited to reporting obligations under the Fixing Long Term Care Act 2021 and other relevant legislation.
  4. This commitment aligns with our dedication to advocating for the client’s best interests and ensuring their safety and quality of care while maintaining transparency and compliance with all relevant regulations.
